UnitedHealthcare (UHC) has announced that, effective August 3, 2025,
OneTouch® will no longer be a preferred brand for diabetes supplies. A
letter (sample attached) was mailed the week of July 7, 2025, to the 582 UC
Medicare Choice members currently using OneTouch® supplies. UHC will also
notify their network pharmacies about this change.

Beginning August 3, 2025, the preferred brands for diabetes testing
supplies will be Accu-Chek® (currently available) and Contour® (newly
added). Members who are unable to switch to one of these preferred brands
may request an exception. The following supplies will be covered at a $0
copay:


   - *Contour® Products *
      - Contour® Plus Blue meter
      - Contour® Next Gen meter
      - Contour® Next One meter
      - Contour® Next EZ meter
      - Contour® Plus test strips
      - Contour® Next test strips
   - *Accu-Chek® Products *
      - Accu-Chek® Guide Me meter
      - Accu-Chek® Guide meter
      - Accu-Chek® Guide test strips

*Frequently Asked Questions (FAQ)*

*What is changing?* As of August 3, 2025, OneTouch® meters and test strips
will no longer be covered as preferred diabetic supplies. The new preferred
brands will be Contour® and Accu-Chek®.

* Why is this changing?* UnitedHealthcare has negotiated lower pricing for
Contour® and Accu-Chek® diabetic testing supplies, allowing members
continued access to necessary supplies at no out-of-pocket cost. The only
difference members will see is the brand of their supplies.

* Will members be notified?* Yes. All members currently using OneTouch®
supplies will receive a letter detailing the coverage change and explaining
next steps.

* What do members need to do?* To maintain $0 copay coverage for meters and
test strips, members must obtain a new prescription for a Contour® or
Accu-Chek® meter and test strips from their provider. Providers may also
contact the member’s pharmacy directly with the updated prescription.

Members may continue using their current meter and test strips. Refills
before August 3, 2025, are encouraged. After this date, OneTouch® products
will be subject to full retail pricing.

* Is prior authorization required?* No. Prior authorization is not required
for preferred products. For non-preferred or non-covered products, members
will need to request an exception.

* Can members request an exception?* Yes. Members or their healthcare
providers may submit a request for an exception if switching to a preferred
brand is not possible.
