UWP Lecturers plagiarism reports

I agree with Kate, but let me add something.  I have had opposing experiences with this.  I have had students who saw the evidence - kind of hard to ignore safeassign - and admitted responsibilty quickly, signed the form and made no problems for me.  

 

On the other hand, at the end of last quarter, I had a case that I didn't find until finals week when I was grading the last essay.  I also wasn't planning to be on campus again until the next quarter began and didn't want to make a special trip just to meet a student who had cheated in the first place (I was bitter about going out of my way for this student who hadn't even bothered to write his own paper - call me crazy).  Anyway, I emailed the student, told him that I'd found the plagiarism and had proof for it, reported that I had filled out the report and had given it to the student conduct office, and called it a day.  I didn't get signature, I left that section of the form blank, and I copied the email I had sent and attached it to the form.  I haven't heard anything negative back from the Student Conduct Office, so I assume what I did was fine.  

 

I did use the grade delay and changed it to an F when the Student Conduct Office got back to me.

I see by the academic misconduct report form that we are now REQUIRED to obtain a student signature on it affirming that we have communicated appropriately with said student.
 
How have other people been dealing with this?  To what lengths must we go to chase down students at the end of the school year?  It seems to me this should not be our obligation.
 
Do they accept these forms without a student signature?
 
Seems like an excellent way for the student conduct office to cut down on their workload.
