If you're on the Creative Writing side, this is what you need to do:

Write a Letter of Intent and, with a copy of your CV (that highlights
relevant work), give both to Andrew Winer.

If there's more information, I'll let you know.  There MAY be a form, but it
isn't written yet, so do this for now.
