[Bgsa] Cookbook recipe e-form

I was asked today at the coffee half-hour if recipes for our Fundraising 
Cookbook need to be hand written on the forms in the office.  You 
actually have three choices:

1.  You can hand write the recipe on the forms available from the office.

2.  You can attach a photocopy to the forms available from the office. 
You can attach as many different recipes as you want to a form, just 
fill out the personal information at the top, and suggest a name of the 
book at the bottom (supplier of the chosen name gets a free cookbook).

3.  You can used the attached e-form and and tab through and fill in the 
various fields and them e-mail the forms back to me.

It's just that simple!!
