Registration
To register to attend either of the flu shot clinics and receive a free flu shot:

  *   You must fill out an Inactivated Vaccine Universal Consent Form 2021<https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2021> - Spanish version<https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2021-spanish>.
     *   Please be sure to print and bring the completed consent form to the flu shot clinic for an expedited experience.
  *   Supplies are limited - register at the Flu Shot Clinic registration website<https://ucriverside.az1.qualtrics.com/jfe/form/SV_9vRQPPUoF2zdKHI>.
  *   While you are strongly encouraged to register to guarantee a flu shot, walk-ins are welcome.
