[Wellnesscommunications] Free Annual Flu Shots on Campus - Tuesday, 9/26 and Thursday, 9/28

Dear Wellness Program Participants:
     As the flu season approaches, we understand how important it is to stay healthy and safeguard ourselves from preventable illnesses. That's why we are excited to announce that Kaiser Permanente is offering FREE flu shots to all members of our community!
Why get a flu shot?
The flu can be a serious and highly contagious respiratory illness that affects millions of people each year. By getting a flu shot, you can protect yourself and those around you from the flu's unpleasant symptoms, potential complications, and avoid missing school or work due to illness. It's a simple yet effective way to boost your immunity and stay healthy throughout the flu season. HR Benefits and Wellness will be hosting the annual Flu Shot Clinics on campus. Kaiser Permanente is sponsoring the flu shot and it is free for UCR Faculty, Staff, Emeriti, and Retirees and the event will be located in an open-air environment at the center for campus.

Registration
You may use the information below to register to attend either day to receive a free flu shot:

  *   New this year, complete the UC Flu Shot Certification form<https://hr.ucr.edu/document/flu-vaccine-certification> to record vaccination was received.
  *   Please complete the consent form found at this link Inactivated Vaccine Universal Consent Form 2023<https://hr.ucr.edu/sites/default/files/2023-08/albertsons-consent-form-english-fillable-2023.pdf> / Spanish version<https://hr.ucr.edu/sites/default/files/2023-08/albertsons-consent-form-spa-fillable-2023.pdf>.
     *   Please be sure to print and bring the completed consent form to the flu shot clinic for an expedited experience.
  *   Flu shot serum supply is limited and issued until exhausted - register at the Flu Shot Clinic registration website<https://ucriverside.az1.qualtrics.com/jfe/form/SV_5swZpVqfjG8f64K>.
  *   While you are strongly encouraged to register to guarantee a flu shot, walk-ins are welcome.
Immunization documentation options

  *   All participants are encouraged to take a cell phone picture of the registration form and retain as proof of receiving the flu shot.
