UnitedHealthcare – Health Planner Mailer to be sent on April 15, 2023

UnitedHealthcare (UHC) has announced that they will be emailing or mailing an 8-page health planner to enrolled UC Medicare Choice members on Saturday, April 15, 2023. The planner will include personalized care checklists and a list of preventive screenings and tests that are recommended. A health planner sample can be viewed at: https://live-ucr-retirees.pantheonsite.io/document/2023-health-planner-mailing.

The goal of the planner is to encourage members to review their Care Checklist and complete the recommended preventive screenings, treatment, and care to maintain good health. The checklist will include useful phone numbers and links as well as the following services:

·         Annual wellness or physical exam

·         HouseCalls visit

·         Managing blood pressure and blood sugar level

·         Colon and breast cancer screening

·         Eye health checkup

·         On-demand emotional help
Members can also access their online account to review their Care Checklist anytime.
