UnitedHealthcare - Name Change from LogistiCare to ModivCare

UnitedHealthcare has announced that effective January 6, 2021, LogistiCare has changed their name to ModivCare. Please note that this was a name change only and does not affect any of the services previously available under LogistiCare.

Information on ModivCare

*       Provides post-discharge transportation to members who are enrolled in the UC Medicare Choice plan administered by UnitedHealthcare


*       Arranges unlimited rides for up to 30 days upon referral from a UnitedHealthcare Advocate and immediately following inpatient hospital discharges or skilled nursing facility stays
