Please be reminded in March 2025, UCOP initiated an audit of dependent eligibility for the UC health and welfare plans.  Various members with participating dependents are required to reverify that your dependents are still eligible for coverage on a UC medical, dental, vision or other UC sponsored plan.  It is important to respond to this request or your dependents will be removed from coverage.

Now through December 31, 2025, UC will conduct monthly eligibility reverification audits of some enrolled family members.  Relationship statuses change over time, which is the reason UC will verify the continued eligibility of certain previously verified family members.  Reverification is in addition to the standard family member eligibility verification (FMEV) process for newly enrolled members.

FMEV Administered by UnifyHR
UnifyHR is the UC Third Party Administrator and manages the verification and reverification process for UC.  They will send requests to the homes of Faculty and Staff members required to reverify dependents on their medical plan (dependents include spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward or overaged disabled child).

Action Required
Respond by the deadline specified in the document or your dependents' medical coverage will be canceled.
