This message outlines the methods by which employees may submit their elections during Open Enrollment (OE).

Open Enrollment Options

1) Self-Service Elections

Open Enrollment Events will be available for employees starting 8:00 a.m. on Thursday, October 26, 2023, and ending at 5:00 p.m. Friday, November 17, 2023 (Pacific Time).  We strongly encourage use of the self-service option by employees during the Open Enrollment period.

Events are configured by benefit eligibility level; therefore, they only allow election of plans for which employees are eligible, which improves accuracy and allows for faster processing.

2) Benefits eForm

The Benefits eForm will be included under the Open Enrollment category in the UCPath Forms Library. Employees may select "Benefit eForms: Open Enrollment" and choose "2024 Open Enrollment" from the Reason for Request option. Once this selection is made, employees will have the option to choose from the following categories:


  *   Faculty/Staff Benefits
  *   Graduate Student Benefits
  *   Medical Resident Benefits
  *   Postdoctoral Scholar Benefits
Each benefit program selection will open a Benefits eForm for the employee to add dependent(s) and make enrollment changes for plans that are available through the Open Enrollment period. Unlike the self-service option, the Benefits eForms are not configured by eligibility level, so all available plan options will display.

3) PDF Form

Employees will have the option to submit an Open Enrollment PDF form. The OE forms for each benefit program, as well as the Leave without Pay options, will be housed in the UCPath Forms Library.

There are three ways to submit the completed/signed PDF form to UCPath:

Submit an inquiry to UCPath: http://ucpath.universityofcalifornia.edu



*       Log in using Single Sign On (SSO)

*       Click "Ask UCPath"

*       Select "Submit an Inquiry"

*       Use the following drop-down selections:
Topic: Open Enrollment
Category: Enrollment Form

*       Subject: Type in "OE PDF Form - 2024 Elections"

Email to UCPath: ucpath at universityofcalifornia.edu<mailto:ucpath at universityofcalifornia.edu>

*       In the subject line, write: "OE PDF Form - 2024 Elections"

*       If the subject line does not read "OE PDF Form -2024 Elections," the form processing may be delayed.

For Leave Without Pay Open Enrollment elections, employees may follow the processes listed above but should use the subject line: "LWOP OE Form Submission."
NOTE: Forms received via UCPath Inquiry or e-mail after 5 p.m. (PT) on Friday, November 17, 2023, will be considered late enrollments and will require review for exception processing. Forms that are mailed, must be postmarked no later than Friday, November 17, 2023.
