UCPath Center Scheduled Maintenance Affecting 1095 Form Access


DBRs: Please share this very important information via your internal department listserv.

On Wednesday, April 5, 2023, UCPath will undergo maintenance that will affect active and former employee access to the 1095 Tax Forms for 2022. The current Forms will be replaced with Forms that are reformatted to meet accessibility standards.  Active and former employees should not experience any changes to the information contained in the Forms.

The 1095 Tax Forms will be unavailable in UCPath on April 5, 2023, from 1 AM - 8 AM Pacific Time. If employees and/or former employees require access to their 1095 during this time, they should be advised to download a copy of their 1095 prior to the scheduled maintenance. As a reminder, 1095 Tax Forms are informational and for member records only.  They are not to be filed with tax returns.
