UCPath – Forms Library Has Been Updated to Display by Employee Categories
In an effort to reduce the submission of incorrect forms and as a follow-up to location feedback on eBenefit Forms, the UCPath Forms Library has been updated to display benefit forms by employee categories.

The library will now include 4 different categories:

·       Faculty/Staff Benefits

·       Postdoctoral Scholar Benefits

·       Medical Residents Benefits (Pending forms)

·       Other Benefits Forms

The following changes are included in the rearrangement of forms:

·       eBenefit Forms are under the Faculty/Staff Benefits category. At this time, only faculty and staff should be utilizing this functionality.

·       The link for Evidence of Insurability under the Faculty/Staff Benefits category was updated to direct employees to the correct pages in UCNet.

·       The Late Enrollment Request Form is part of the Postdoctoral Scholar Benefits category.

·       The Medical Residents Benefits category (Pending forms).

·       Other Benefits category includes miscellaneous forms such as the Dependent Information Update Form and Declaration of Tax Dependency (UPAY 886).

You may review the forms under each category by accessing the Forms Library on the UCPath Dashboard.
