Anthem – Transition to Lark for the Diabetes Prevention Program

Anthem will transition its Diabetes Prevention Program (DPP) contract with Solera to the new vendor, Lark effective January 1, 2023.

The transfer will begin in September 2022:


  *   Members who want to join the Diabetes Prevention Program will be enrolled in the Lark program.
  *   Any UC Medicare Supplement member who is currently with Solera will either graduate by December 31, 2022, or be switched to Lark on January 1, 2023. Any non-Medicare member with Solera needing care beyond 2022 will also be moved effective January 1, 2023.
  *   The Anthem.com and the Sydney Health Mobile App will begin to point members to Lark effective September 1, 2022.
