Anthem – Prime Healthcare Leaves the Anthem Network
The UC Office of the President (UCOP) was recently notified that Anthem has severed their contractual relationship with Prime Healthcare (PHC). As a result, PHC facilities are no longer in the Anthem network. This decision was suddenly made by Anthem—UC had no prior knowledge of the termination.
The cancellation date of each hospital’s network status is listed below. It is very important to note that most of the terminations are retroactive. However, services incurred between July 23, 2021 and September 16, 2021, will be processed as in-network. Anthem Health Guide (1-844-437-0486) has been briefed on this change and are ready to answer inquires.

Termination retroactive to July 23, 2021

        ·        Desert Valley Hospital – 0

        ·        Shasta Regional Medical Center – 4

Termination effective September 16, 2021

        ·        Centinela Hospital Medical Center – 11

        ·        Chino Valley Medical Center – 2

        ·        Encino Hospital Medical Center – 5

        ·        Garden Grove Hospital and Medical Center – 1

        ·        Huntington Beach Hospital – 0

        ·        La Palma Intercommunity Hospital - 2

        ·        Paradise Valley Hospital – 3

        ·        Sherman Oaks Hospital – 12

        ·        West Anaheim Medical Center – 2

Anthem-initiated termination (date to be determined)
