RE:  New UPAY 850 forms available

 

We have recently received some outdated UPAY 850 forms dating back as far as 2001!  Please go through your files and recycle any UPAY 850 forms dated previous to 10/09. There have been notable changes to the medical and dental plans, family member information, as well the structure of the form itself within the last eight years.

 

Forms can be ordered from the Fulfillment House.
