Two applications on the At Your Service Online (AYSO) website will be closed
temporarily to allow UCOP to complete the year-end health and welfare
changes.  AYSO's Benefits Enrollment (aka New Hire) and Current Enrollments
applications is closed now and will re-open on Wednesday, January 5, 2011.

Employees using Current Enrollments will continue to view their 2010 Health
& Welfare enrollment (Open Enrollment changes will not be reflected).
Employees will be able to view their 2011 enrollment (including their Open
Enrollment changes) applications on or after January 5, 2011.  This downtime
will not affect one's  ability to access earnings statements, view and print
prior W-2s, enroll in direct deposit or change addresses. 

An employee in their Period of Initial Eligibility (PIE) who did not
complete their benefits enrollments by 5 p.m. on December 10, 2010 on the
AYSO website will need to complete a UPAY 850 form and submit to the
Benefits Office within their 31-day PIE deadline.  The UPAY 850 forms can be
mailed to the Benefits Office or faxed to (951) 827-2192.  The UPAY 850 form
