The HR Benefits and Wellness Offices will be hosting the annual Flu Shot Clinics at UCR. This exclusive offer, sponsored by Kaiser Permanente, is free for UCR Faculty, Staff, Emeriti & Retirees and provided in an open-air environment at the center of campus.

What You Need to Know

The Flu Shot Clinics will take place as follow:

  *   Tuesday, September 26, 2023 | 11 a.m. - 3 p.m. | UCR Campus Bell Tower North<https://campusmap.ucr.edu/?find=P5530>
  *   Thursday, September 28, 2023 | 11 a.m. - 3 p.m. | UCR Tomás Rivera Library Walkway<https://campusmap.ucr.edu/?find=P5322>
Attend and receive:

  *   Free annual flu shot
  *   Giveaways
  *   The first 25 flu shot recipients will receive a gift
  *   Healthy snacks
What You Need to Do

To register to attend either day to receive a free flu shot:

  *   Complete the UC Flu Shot Certification form<https://hr.ucr.edu/document/flu-vaccine-certification> to record that the vaccination was received.
  *   Complete the consent: Inactivated Vaccine Universal Consent Form 2023<https://hr.ucr.edu/sites/default/files/2023-08/albertsons-consent-form-english-fillable-2023.pdf> / Spanish version<https://hr.ucr.edu/sites/default/files/2023-08/albertsons-consent-form-spa-fillable-2023.pdf>.
  *   Print and bring the completed consent form to the flu shot clinic for an expedited experience.
  *   Register at the Flu Shot Clinic registration website <https://ucriverside.az1.qualtrics.com/jfe/form/SV_5swZpVqfjG8f64K> - the flu shot serum is limited and will be administered until it is exhausted.
  *   While you are strongly encouraged to register to guarantee a flu shot, walk-ins are welcome.
