The HR Benefits Office and Workplace Health and Wellness will be sponsoring Flu Shot Clinics at UCR. This exclusive offer is free for UCR Faculty, Staff, Emeriti & Retirees and provides convenient access with multiple locations on campus.

What

*       Face coverings required

*       Social distancing observed

*       Giveaways

*       Raffle prizes

*       Healthy snacks

*       Water

*       Post and distribute the Flu Shot Clinics flyer which can be viewed on UCR's Human Resources webpage at: https://hr.ucr.edu/document/flu-shot-clinic

When

*       Tuesday, September 28, 2021 from 9:00 a.m. - 1:00 p.m. located at the UCR Bell Tower. The location can be viewed on UCR's campus map at: https://campusmap.ucr.edu/?find=P5530

*       Wednesday, September 29, 2021 from 12:00 - 4:00 p.m. located at Pentland Hills - Fox Hole (Room F111/G101). The location can be viewed on UCR's campus map at: https://auxiliarydocs.ucr.edu/housing/map/community-map_PH.pdf

Registration
To register to attend either of the flu shot clinics and receive a free flu shot:

*       You must fill out an Inactivated Vaccine Universal Consent Form 2021 which can be viewed at: https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2021. The Spanish version can be viewed at: https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2021-spanish.

o   Please be sure to print and bring the completed consent form to the flu shot clinic for an expedited experience.

*       Supplies are limited - register at the Flu Shot Clinic registration website at: https://ucriverside.az1.qualtrics.com/jfe/form/SV_9vRQPPUoF2zdKHI.

*       While you are strongly encouraged to register to guarantee a flu shot, walk-ins are welcome.
