The HR Benefits Office and Workplace Health and Wellness will be sponsoring Flu Shot Clinics at UCR. The Flu Shot Clinics flyer can be viewed at: https://hr.ucr.edu/sites/default/files/2022-09/flu-vaccination-clinic_vertical2.pdf. This exclusive offer is free for UCR Faculty, Staff, Emeriti & Retirees provided in an open-air location, at the center of campus, near the Bell Tower.

Registration
You may use the information below to register to attend either day to receive a free flu shot:

*        Please complete the consent form found at this link: https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2022. The Spanish version can be viewed at: https://hr.ucr.edu/document/inactivated-vaccine-universal-consent-form-2022-spanish.

o   Please be sure to print and bring the completed consent form to the flu shot clinic for an expedited experience.

*        Flu shot serum supply is limited and issued until exhausted - register at the Flu Shot Clinic registration website at: https://ucriverside.az1.qualtrics.com/jfe/form/SV_0PuzCZisdPu1ANg.

*        While you are strongly encouraged to register to guarantee a flu shot, walk-ins are welcome.
Immunization documentation options

*        Kaiser Members may note their Medical Record Number on registration form and log in to their member account to print documentation.
Non-Kaiser Members are encouraged to take a picture of the registration form as proof of the flu shot.
