Updated Navitus Transition Decision Grid – April 2022

The Navitus Transition Decision Grid has been updated to include the addition of the Personal Member Transitions section for Anthem plan members. For more information, view the Navitus Transition Decision Grid with highlighted changes at: https://hr.ucr.edu/sites/g/files/rcwecm656/files/2022-04/navitus-transition-decision-grid.pdf.

Under the Medicare section, the new information adds clarity to medications requiring prior authorization or step therapy, and medications that will be on a higher tier.

The COVID-19 section for Medicare members has also been expanded. The transition grid gives more details with changes highlighted in yellow.

Members may obtain assistance with questions about the Navitus transition from the Customer Care team by calling (855) 673-6504 for non-Medicare members and (866) 270-3877 for members with Medicare; available 24/7 except for Thanksgiving and Christmas.
