The annual Form 1095C form documenting Employer-Provided health insurance offer and coverage is now available electronically beginning Friday, January 30, 2026. Instructions for Former Employees: Former employees may access and download the 1095-C form for up to three years following separation from employment. After logging in, navigate to Income & Taxes and select the corresponding tile for the desired form.
Please keep this timeline in mind should you receive inquiries from employees or former employees.
Important security reminder:  The University of California does not send W-2 statements by email or text. If an individual receives an email or text with a link or an attachment to view a W-2 statement, it is a phishing scam designed to gain private information. Please advise employees not to open attachments or click email links claiming to provide access to W-2 statements.
