The UCPath Center offers the following correction regarding the FSA eForm submission deadlines and final paycheck dates for biweekly and monthly employees and apologizes for the oversight. In the previous communication, the dates were inadvertently reversed.
The Corrected Information is to be used for submission deadlines:

UCPath - Self-Service FSA Elections will be unavailable from 10-01-2025 to 11-30-2025. UCPath is carefully managing year-end FSA enrollments to ensure accuracy and prevent financial challenges. Employees who are newly hired, newly eligible, or have a qualifying life event between October 1 and November 30, 2025, cannot make FSA elections or updates for 2025 via the self-service portal.
However, eligible employees may still enroll in or update their current FSA elections by submitting a Benefits eForm that corresponds to their qualifying event.
How to Make 2025 FSA Plan Elections or Updates from October 1 - November 30
Accessing the Benefits eForm via UCPath
To submit the FSA enrollment or update, employees should follow these steps:
1. From the main menu, select Benefits & Retirement
2. Under the Forms category, choose Benefits Request eForm
3. Click Submit under the Benefits eForm Hub
4. Select the Event Date that matches their qualifying event
5. Choose the appropriate Employee Benefit Category
6. Use the Reason for Request drop-down to select the applicable reason
7. In the textbox labeled "Please Explain Why You Are Using This Form," enter:
FSA Enrollment for 2025
8. Complete the form to make the enrollment updates
9. Review and agree to the Terms and Conditions and Signature Acknowledgment
10. Click Submit to send the form to UCPath for processing

Note: FSA coverage begins on the first day of the month following enrollment, subject to payroll processing deadlines. The employee's elected FSA contribution will be distributed across the remaining pay periods in 2025. These elections do not carry over into the next plan year. To maintain coverage, employees must submit new FSA elections during Open Enrollment for the 2026 plan year.
Self-Service Availability
Self-service functionality for FSA elections will resume on December 1, 2025. Any FSA enrollment submitted via self-service in December 2025 will apply to the 2026 calendar year.
Submitting Enrollment Forms (PDF)
