Kaiser Permanente - 2023 1095-B Distribution Change

DBRs, please share this information via your internal listserv.

Kaiser Permanente is changing the distribution of its annual 1095-B forms. While these forms are not necessary when filing tax returns, forms for 2023 will be emailed automatically to members who have registered on KP.org and opted for electronic notifications.

Kaiser members who have not registered must visit the personal documents page on KP.org, or call the Members Services Contact Center at 1.844.477.0450 to request a copy.
