Effective July 25, 2023, an updated Disability Self-Identification form will be available to UC employees via UCPath self-service and to applicants via Candidate Gateway.

These updates aim to improve form completion rates with new language, formatting and disability options. Please share information about the updated form throughout your location as appropriate.



What's Changing?



The improved form includes the following newly listed disabilities:

*       Alcohol or other substance use disorder; not currently using drugs illegally

*       Disfigurement, for example, disfigurement caused by burns, wounds, accidents or congenital disorders

*       Mobility impairment, benefiting from the use of a wheelchair, scooter, walker, leg brace(s) and other supports

*       Neurodivergence, for example, attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder, dyslexia, dyspraxia and other learning disabilities

*       Partial or complete paralysis; any cause

*       Pulmonary or respiratory conditions, for example, tuberculosis, asthma, emphysema

*       Short stature/Dwarfism

*       Traumatic brain injury

Why is the form changing?

UCPath will implement the updated form by July 25, 2023, to ensure full compliance with federal regulations.



Who initiated the change?



The Office of Management and Budget<https://www.whitehouse.gov/omb/> (OMB) approved the Office of Federal Contract Compliance Programs<https://www.dol.gov/agencies/ofccp> (OFCCP) revised form<https://www.dol.gov/agencies/ofccp/self-id-forms> on April 25, 2023, and it will remain valid until April 30, 2026.
