Anthem Blue Cross - PPO ID cards for 2023



Beginning January 1, 2023, Accolade replaces the Anthem Health Guide as the member services resource.  As a result, many of the phone numbers on the back of all ID cards will change next year.  All members will receive new ID cards and new group numbers will also be shown on these ID cards.



For your reference, below are samples of the new member ID cards for the CORE, UC Care, and Health Savings Plan medical plans.
