This grid provides a description of the different scenarios that Anthem PPO members may experience when one or more of their prescription drug(s) are affected by the change in the Pharmacy Benefit Manager from IngenioRx to Navitus Health Solutions.

The Navitus Rx Transition Decision Grid also describes how Navitus will contact its members when there are specific changes to the various scenarios in the grid.
