The Human Resources/Benefits Office is still trying to encourage employees
to add or change their beneficiaries online. For the University Retirement
Plan and Capital Accumulation Provision (UCRP/CAP), please have employees go
online at http://atyourservice.ucop.edu <http://atyourservice.ucop.edu/>
and after July 18th for 403(b), DCP and 457 Plans, have them go to the
FITSCo website at www.fidelity.com/atwork.   However, if an employee still
wishes to use a form, please be aware that the Human Resources/Benefits
Office in Oakland has made a few changes to the following beneficiary forms
so that they will be ready for the transition of the 403(b) and DCP
recordkeeping to FITSCo in July. 

 

The revised forms are now available on At Your Service
(http://atyourservice.ucop.edu <http://atyourservice.ucop.edu/> ) and will
be available through the KP fulfillment house. Please discontinue using any
UBEN 116 or UBEN 117 forms with a revision date earlier than 5/05. The
revision date is printed on the upper left corner of the cover sheet.
Please send all forms directly to the address on the form. 
 
(UBEN 116) Designation of Beneficiary Employees -- UCRP and Capital
Accumulation Provision and UC-Sponsored Insurance Plans
 
(UBEN 117) Designation of Beneficiary Retirees, Former Employees and Others
-- UCRP and Capital Accumulation Provision
 
UBEN 116:
 
This form is used by employees to designate beneficiaries for UCRP and CAP. 
 
Previously, this form was also used to designate beneficiaries for the
403(b) and DC plans. To designate beneficiaries for the DC, 403(b) and
457(b) plans, employees will now contact FITSCo (Fidelity Investments
Tax-Exempt Services Company). 

The form includes information on how to contact FITSCo. 

IF YOU DO NOT NAME A BENEFICIARY section: The definition of "child or
children" has been updated to be consistent with plan regulations. 

We have deleted the Tax-Deferred 403(b) and Defined Contribution Plans,
since designation of beneficiaries for these plans and the 457(b) plan will
be done through FITSCo. 
 
UBEN 117:

This form applies to UCRP/CAP and is used by retirees, former employees and
others with Qualified Domestic Relations Orders (QDROs). 
 
The changes listed above for the UBEN 116 have also been made to this form.
