The annual Form 1095C form documenting Employer-Provided health insurance
offer and coverage is now available electronically beginning *Friday,
January 30, 2026*.  Employees may view and access forms directly in the
UCPath

& Taxes* and select the corresponding tile for the desired form.

Please keep this timeline in mind should you receive inquiries from
employees or former employees.
