Re:          UCR COVID-19 Symptom Monitoring and Reporting Procedures for
Covered Employees and Supervisors

On Monday, July 27, 2020, Human Resources released the UCR COVID-19 Symptom
Monitoring and Reporting Procedures for Covered Employees and Supervisors.
For the past month, a coordination of teams across campus worked diligently
to provide a resource for employees and supervisors to understand the
purpose of the Qualtrics System: UCR Symptom Monitoring Survey and to
ensure individuals understand their responsibilities as it relates to
reporting and sharing information.



The document includes information on:

·       UCR Symptom Monitoring

·       Confidentiality

·       Completing the Survey

·       Certificate of Clearance

·       Procedures

·       Notification of Potential Exposure
