[Corebotanyfaculty] Update - Daily Wellness Survey No Longer Needed

Good morning,

As of today, staff on campus will not be required to complete the Daily Wellness Check.  They have changed the site to now direct you to the COVID Screening Check.  The only time you are now to go to the survey site is to report a positive test, symptoms, exposure or reporting required tests.

Dear DA's,

Please be advised that the COVID Daily Wellness Check has just been updated and is now the COVID Screening Check<https://ucriverside.az1.qualtrics.com/jfe/form/SV_0pOB27zdrdkYxzE>. When you go to the link, you will be automatically directed to the COVID Screening Check.  As it indicates and as a reminder, highlighted below, the survey is no longer required to be completed daily, but only for the reasons indicated (positive COVID test, symptoms, exposure, and reporting required tests).

This change begins today so please make sure to communicate that to your departments. Reach out with any questions you may have.
