Per state guidance and with the COVID-19 Public Health Emergency (PHE) having ended, beginning November 12, 2023, the Postdoctoral Scholars Health Net PPO plan will resume normal health plan coverage for certain COVID-19 benefits.

PPO Plan:
What is changing?

  *   COVID-19 vaccines, testing, and therapeutics received from an out-of-network provider will be subject to plan benefits. No prior approval is required.
  *   Over the counter (OTC) COVID-19 kits purchased at an out-of-network provider pharmacy are subject to the plan's lab office visit copay. A medical claim form will need to be submitted*.

What's not changing?

  *   COVID-19 vaccines, testing and therapeutics received from an in-network provider remain at no cost. No prior approval required.
  *   OTC COVID-19 Test kits are at no cost if you use one of Health Net's in-network retail pharmacies*.

*Maximum of eight tests per covered member in a 30-day period. The maximum reimbursement for each test per member is $12, includes costs such as taxes and shipping fees.

If members have any questions, they should contact Health Net's Customer Contact Center at the phone number listed on the Member ID card.
