[CaTEACH-SMI] Reminder...SMI's Test Reimbursement Program

Attention SMI Students:
If you took the CSET Exam in January, you should have received your 
test scores by now.  As you review your exam results, consider 
participating in SMI's test reimbursement program...

Submit the following items to the SMI Resource Center
(1104 Pierce Hall) to request for reimbursement

1.           Completed W-9 form
http://smi.ucr.edu/pdf/w09form.pdf

2.           Original Receipt for Test

3.           Test Scores

For verification purposes, SMI will gladly make copies of the 
original documents.
**Passage of the exam is not required to become eligible for reimbursement.**
