Dear DA's,

I have heard from many of you and your employees asking about the COVID booster exemption process. Employees who want to request a new exemption can fill out the survey here: Exemption Request (Employees)<https://ucriverside.az1.qualtrics.com/jfe/form/SV_9p2q5OtG0bj0Oeq>

Sedgewick is going to be the entity responsible for reviewing and processing booster exemption requests. However, they are still working out the process with campus for receiving the exemption requests for boosters and the criteria to use in reviewing them. If you or your employees need to request a medical or religious exemption, go through that link and submit your information. Please understand that it may take time to receive a response from Sedgewick as the process is still being determined.

Employees who already had religious or medical exemptions without an end date on file do not need to request a new exemption to the booster. This process is only needed for employees who have a new need for religious or medical exemption.

If you have an employee who is working 100% remote, please send me their signed remote work agreement. I am placing those employees on a tracking spreadsheet so that they are removed from the non-compliance list if they have not completed the vaccine/booster process.

I have been asked about the booster exemption by many employees in the past month so please share the link broadly with your teams and let them know they may experience a delay in hearing from Sedgewick.
