As many of you may know, it’s performance appraisal season for staff. In support of this process, I’m kindly requesting your feedback on the staff members you work with regularly.
Please submit your feedback to me by Friday, April 11th.

While you’re welcome to use it, it’s not required, a brief few sentences or paragraph via email works just as well.
