Chancellor Wilcox and Vice Chancellor/Dean Deas announced last week, all employees who are working in campus owned or leased facilities are required to complete the daily UCR Symptom Monitoring Survey linked below starting this week. Employees working remotely part of the time are required to complete the survey only on all days when they will be working in campus facilities. Please complete this task before coming to work. The survey should take about 30 seconds to complete. CERTIFICATE OF CLEARANCE
- If the survey provides you with clearance to work, you will receive a certificate via email.
- This certificate expires at 4:00 AM the day after the survey is taken.

ADDITIONAL GUIDANCE
- If you are unable to complete the survey before coming to work, please make arrangements with your supervisor to complete the survey immediately upon your arrival.
- If you have general questions about your symptoms or COVID-19, please call your primary care physician.
- If you have severe symptoms such as confusion, severe dehydration or worsening difficulty breathing, go to the ER immediately and/or dial 9-1-1.
- If you are unsure if you should go to the ER, you can call your primary care physician to schedule an urgent Telehealth visit.

July 17, 2020: A staff member who works off-site.
July 16, 2020: A staff member who has not worked on campus since March 13th.
July 15, 2020: A staff member who has not worked on campus since July 10th.
July 14,2020: A staff member who has not worked on campus since July 9th.
July 10, 2020: A staff member who has not worked on campus since July 7th.
July 9, 2020: A staff member who has not worked on campus since July 7th.
July 7, 2020: A staff member who has not worked on campus since July 1st.
July 6, 2020: A staff member who has not worked on campus since June 26th.
June 30, 2020: Four staff members; one has not worked on campus since March 13th; one since June 28th; two since June 26th; and one since June 24th.
June 29, 2020: A staff member who has not worked on campus since June 24th.
June 26, 2020: Two staff members; one has not been on campus since June 16th, the other since June 24th.
June 18, 2020: A staff member who has not worked on campus since June 10th.
