a memorandum regarding the campus’ General Liability and Employment
Practices Insurance rates was distributed to you. In that memorandum, it
stated that certain DOS codes were excluded from the rate assessment. It has
recently come to my attention that the DOS codes related to Fellowships (FEL, <st1:place
w:st="on">FES</st1:place>, FEN) are no longer in use and can be deleted from
that list<o:p></o:p></span></font></b></p>

the Incentive Award Payment program (DOS codes - IIA, TIA, MIA, PDA, SPA) was
discontinued as of July 1, 2005 and a new program, the Staff Recognition and
Development Awards for non-represented employees is now in effect. The new DOS
code for this new program is SRD. <o:p></o:p></span></font></b></p>
